Please read the following information carefully before submitting this application.

I give my child permission to attend ISG Sunday School. I give full responsibility of my child's care during the hours to ISG Sunday School staff and allow my child to participate in all ISG Sunday School activities.

ISG Sunday School Rules & Regulations

Do:

My child will participate fully in all activities and sessions.
I will immediately report any illness or injury to a member of staff.
I will report any concerns regarding health and safety to a staff member.
I will report sightings of any suspicious persons to a teacher or administrator.
My child will dress appropriately. Boys and girls must dress modestly. For girls, a hijab is always recommended.
I will inform a teacher or an administrator if my child is unable to attend on a particular day.
I will ensure that my address and contact deatils on the parent's portal is alaways current. Additionally, I agree to contacted if my child becomes ill while attending ISG Sunday School or if there ia an emergency.
I will notify a teacher or an adminsitrator if I am going to be late to pick up your child.

Do not:

Neither my child or I will bring dangerous or harmful objects to ISG Sunday School.
My child will not leave the ISG Sunday School premises without permission and supervision.
My child not participate in dangerous activities (e.g., crossing roads, jumping off chairs/tables, fighting).

Expected behaviour:

My child and I will follow instructions from ISG Sunday School organisers at all times.
My child and I will maintain honesty at all times.
My child and I will behave with kindness, consideration, and modesty with others.
My child and I will respectly engage with teachers, administrators and other ISG Sunday School staff .
My child will be puntual
My child and I will commit to the following: No arguing, No fighting, No raising voices and No foul language

Failure to adhere to these rules and regulations will result in my child being excluded from learning activities or the ISG Sunday School.

Monthly Fees

Full monthly fees = $25 per child
Sibling discount (2 to 3 children) = $20 per child
Sibling discount (4 or more children) = $60 (based on $75 family fees ceiling and the above sibling discount)

By submitting this form, I affirm that I have read the rules and regulations concerning the ISG Sunday School. I understand that the ISG Sunday School, managed by the Islamic Society of Geelong, reserves the right to change its rules and regulations concerning the Sunday School, and it is my responsibility to maintain compliance with all current rules and regulations.
